The Family Partners Adult Day Specialized Programs

In order to keep a happy, healthy environment, each participant will go through an extensive assessment process. This person centered assessment determines what specific service combination is best suited to meet each person’s needs. This allows your loved one to have a day that is geared towards his or her personal abilities, providing the opportunity to thrive in our setting. Each assessment is provided by a degreed professional and discussed with the family and individual client, as appropriate.
