Adult Day Services Participation

Each program participant must meet our minimum requirement of 2 days of services per week. From there we build a custom package based on individual needs. Highlights include:

• A choice of from 2 up to 5 days of service.
• All fees/rates are explained up front
  • A written contract for services is provided
• Should additional individual personal services be needed, they can be added on an a la carte basis.

  • Dry cleaning drop off and pick up
  • Concierge services
  • Take home meals
  • Nursing services
  • Bathing services
  • Salon services
• Credit card payments accepted.

Affordability for community residents is important to Family Partners Adult Day Services. As a result, we accept many funding sources including Private Pay, Veterans Assistance, Long Term Care Insurance, Alzheimer’s Association Aid, Medicaid and more. Our helpful staff can also assist in applying and securing funding as needed.

Our Person Centered Program Assessment

In order to keep a happy, healthy environment, each participant will go through an extensive assessment process. This person centered assessment determines what specific service combination is best suited to meet each person’s needs. This allows your loved one to have a day that is geared towards their personal abiliities giving them the opportunity to thrive in our setting. Each assessment is provided by a degreed professional and discussed with the family and individual client, as appropriate.
