Family Partners Caregiver Programs

At Family Partners Adult Day Services, it is our desire is to equip families with the tools necessary to enhance the quality of life for their loved one at home. The day to day pressures of caring for a loved one with memory loss associated with dementia or Alzheimer’s disease can be overwhelming. Without support, many caregivers quickly reach their own emotional and physical limits. We offer the following family caregiver programs to all Family Partners families:

  • Caregiver Consultation – Evaluation of what types of support are best for each personal situation: two
    in-depth home visits; written comprehensive strategy plan tailored specifically for your family; three follow up telephone family conferences in the first year and one every six months thereafter
  • Caregiver Training – Customized training, conducted during two home visits, offers simple strategies to help transform the home into a healthy, safe and comforting place; improve communication with the person suffering from memory loss; and provide independence for the person suffering from memory loss
  • Referral and Aftercare – professional advice services; support via telephone and or e-mail; community resources

Family Partners takes care of families' loved ones during the day. For an additional cost, our caregiver training gives caregivers the tools to successfully manage care for those times when they are on their own. Our customized, hands-on-training, in-home consultations, coupled with individualized strategy plans give caregivers the skills to manage daily stress and transform the home into a place of safety and comfort for everyone who lives there.
